1. What is the Apollo Pharmacy medical card?

The Apollo Pharmacy Medical card is a FREE membership card issued by Apollo

Pharmacy, a unit of Apollo Hospitals. With this membership card, you can purchase

medical products at Apollo Pharmacies all over India.

2. What are the benefits of the Apollo Pharmacy medical card?

As a holder of the Apollo Pharmacy medical card, you get a 10% discount on your total

bill value and exclusive benefits (refer to Apollo offer document). More importantly, all

the purchases you make at an Apollo Pharmacy will automatically get reflected in

Cognizant’s MyPay portal on a monthly basis. This means that you can claim your

medical reimbursement without the need for submitting original bills1.

3. Who can apply for this card?

All Cognizant associates can avail benefits under this card. Those who have defined a

medical allowance under the Flexible Benefit Plan will be eligible for reimbursement for

Pharmaceutical purchases made under this program.

4. How do I get the Apollo Pharmacy card?

Apollo Pharmacy medical cards are available at all Cognizant facilities. To know the exact

details for your location, contact your location admin help desk.

5. What are the other steps I need to take to become a member?

After you get the card, log on to the http://203.196.190.90/ and enroll yourself as a

member by providing the details requested on the website. Your membership will be

activated only after the enrollment process in completed.

6. So how does this actually work?

a. Get the card and then update your details on the Apollo site

b. Update the membership card number in https://mypay.cognizant.com

c. You can now make purchases from any Apollo Pharmacy in India from April 1,

2011

d. When you make a purchase, Apollo Pharmacy will share the details with

Cognizant on a monthly basis. The purchases made by you will be classified as

pharmaceutical and non‐pharmaceutical.2

e. This will be uploaded in My Pay and be available for your categorization or

declaration for reimbursement.

f. You have to confirm whether the transactions you have made is to be

reimbursed by selecting ‘Yes’ or ‘No’

g. All pharmaceutical purchases that have been marked ‘Yes’ will be reimbursed to

you subject to your eligibility

h. Payment will be made to your salary account as per the medical payment cycle

as mentioned in the Flexible Benefit Plan.

7. If I opt for the Apollo medical card, am I prohibited from purchasing from other

pharmacies?

No. You can continue to purchase medicines from other pharmacies as well.

8. How do I get reimbursement of the bills from other pharmacies?

The option for submitting original bills for reimbursement is still available for you,

should you choose to purchase medicines from other pharmacies.

9. Can I use this card if I or another member of my family were to be hospitalized?

No. This card cannot be used for hospitalization expenses. This card can be used only for

purchase of medicines.

general items will be classified as non-pharmaceutical products

10. What if I use only a portion of the allocated allowance for that month? What will

happen to the rest of the money?

The rest of the allowance for that month will be carried over to the next month. For

example, if your eligibility is Rs.1250 for April and you purchase Rs.250 worth medicines

for the month of April, then the balance Rs.1000 will be carried over and can be used for

subsequent month purchases.

11. What if I buy medicines worth more than the allowance for that month?

You need to confirm only the amount eligible for reimbursement. If your bill amount is

higher than your eligibility, you may skip the confirmation for the current month and do

it in the subsequent month. For example, if your eligibility is Rs.1250 for April and you

purchase Rs.2000 worth of medicines in one bill, then you should skip the confirmation

for April. You need to wait till May for your eligibility to be Rs.2500 and then confirm for

reimbursement.

12. What if I do not use the membership card at all during the year? How will I get my

medical allowance?

The amount that you are eligible for under medical reimbursement will be credited

along with your salary at the end of the year as special pay after the applicable taxes

have been deducted.

13. What will happen to the current policy of submitting prescriptions for Rs.500 and

above?

If you purchase through Apollo pharmacies, you need not submit prescriptions to us.

14. Can I use the card in Apollo hospitals for clinical and laboratory tests and avail of these

benefits?

No. This card can be used only to purchase medicines from the Apollo Pharmacy
